Insurance

The insurance we accept is subject to change and not all providers accept all plans. Currently, we accept:

Tricare (Active Duty, Retired and Tricare for Life).
VA Community Care

We may show that we are in-network with other insurance companies, we are at capacity and are not currently accepting new clients unless referred by a VAMC or MTF.

Payment

We accept cash, check and all major credit cards as forms of payment.We also accept PayPal payments. Please use the email address: info@evolvinglife.net to remit payment via PayPal or in your respective patient portal.

Cancellation policy

If you are unable to attend a session, please make sure you cancel at least 24 hours beforehand. Otherwise, you may be charged for the full rate of the session.
